Steps to Obtain Evidence of Self-Insurance

1. Download and fill out a Request for Proof of Self-InsuranceOpen PDF.

Tip: The Request for Proof of Self-Insurance is a fillable PDF. Download the file and fill it out on your computer (remember to save your changes).

2. Once you have finished filling out the Request for Proof of Self-Insurance, send the completed form to [email protected]. A representative from the Risk Management Department’s Insurance Compliance Unit will process your form and send a Letter of Self-Insurance to the requester and the certificate holder.
