Field Trip Insurance Compliance

REQUIRED - Out-of-State Waiver: The Out-of-State waiver is a document for out-of-state field trips or excursions and is required by all adults and all parents or guardians of students taking out-of-state field trips or excursions. This should be submitted with your field trip request.

REQUIRED (Each Student) - Transportation Waiver form: This form is used when a parent/guardian gives permission for a District employee/volunteer to transport their child to or from a school-related event, field trip, or activity. (The form can also be used when a parent/guardian chooses to transport their own child to or from a school event/field trip)

REQUIRED (Each Driver) - Authorization for Employee Use of Private Vehicle for Student Transportation and/or Parent/Volunteer Private Automobile Driver Certification of Liability Insurance: This form ensures that the employee/volunteer has proper authorization, insurance, and a valid driver’s license, and it helps the District manage liability and safety compliance.

REQUIRED - Care, Custody, and Control Letter: This letter is used when parents or guardians assume responsibility for arranging the transportation and supervision their own child during a school-related event, field trip, or activity, instead of using school-provided transportation or supervision. (a Transportation Waiver form (English) can also be used when a parent/guardian chooses to transport their own child to or from a school-related event, field trip, or activity)
